PATIENT BILL OF RIGHTS
Facey Medical Group is a caring & innovative team dedicated to enthusiastically improving the quality of life and health of the people we serve.
We believe that you, as our patient, have certain rights:
- Right to receive information about Facey Medical Group, its services, its practitioners and providers, and members' rights and responsibilities.
- Right to be treated with respect and recognition of your dignity and right to privacy.
- Right to participate with practitioners in decision-making regarding your health care.
- Right to candid discussion of appropriate or medically necessary treatment options for your condition regardless of cost or benefit coverage.
- Right to voice complaints or appeals about Facey Medical Group or the care provided.
- Right to make recommendations regarding Facey Medical Group’s member rights and responsibilities policies.
- Right to be represented by parents, guardians, family members or other conservators when unable to participate in your medical treatment decisions.
- Right to access services and information in alternative format (inclusive of oral and written) in the language that is prevalent to the Facey Group population.
We also believe that you, as our patient, have certain responsibilities when receiving care from Facey Medical Group.
- Responsibility to supply information (to the extent possible) that Facey Medical Group and its practitioners need in order to care for you.
- Responsibility to follow the plans and instructions for care that they have agreed upon with their practitioners.
- Responsibility to understand your health problems and participate in developing mutually agreed upon treatment goals to the degree possible.

Facey Medical Group's provision of health services is not influenced by member race, ethnicity, national origin, religion, sex, age, mental or physical disability or medical condition, sexual orientation, claims experience, medical history, evidence of insurability (including conditions out of acts of domestic violence), genetic information, or source of payment...
Compliance with anti-discrimination policy will be monitored by the Corporate Compliance Officer and the Medical Director.

The materials provided to you are guidelines used by the medical group to authorize, modify or deny care for persons with similar illnesses or conditions. Specific care and treatment may vary depending on individual need and benefits covered under your contract. Those are guidelines and do not constitute the standard of care.
- Facey Medical Group will disclose a list of network providers to members, upon request.
- Utilization Management medical clinical guidelines will be disclosed to members and practitioners, upon request.
- All Utilization Management decisions are based on appropriateness of care and service.
- Facey Medical Group/Foundation does not compensate practitioners for individual denials.
- Facey Medical Group/Foundation does not offer incentives to encourage denials.
- Facey Medical Group/Foundation does not have the financial incentives that would encourage decisions that would impact under/over-utilization of care, service or available member benefits.
For questions or concerns that are related to a referral that your provider has submitted, patients can telephone our main Utilization Management number,
818-837-5660. Hearing or speech-impaired members (TTY users) can call
711 relay services. Collect calls are accepted for patient referral matters, and Spanish speaking staff members are available.
