Diabetes & Heart Disease Prevention
Research shows that healthy eating, moderate exercise, and a healthful lifestyle could delay or possibly prevent Type 2 diabetes and heart disease in people who are at risk. We’ll help you identify the risk factors for heart disease and diabetes that you can easily monitor and manage on your own. This class will provide you with tools to make lifestyle changes including improving diet, increasing physical activity, achieving weight loss, reducing stress, and maintaining healthy blood pressure and cholesterol levels.

Diabetes Education Program
A two-session class which will help you maintain better blood sugar control and learn how to live healthier with Type 2 Diabetes. You will learn lifestyle and nutrition strategies for managing your diabetes, about medications and how to avoid or delay diabetes-associated complications.

Gestational Diabetes
During a one-to-one appointment with a registered dietitian, certified diabetes educator you will receive an individualized gestational diabetes nutrition care plan and diabetes management education including managing blood sugars, learning how to test blood sugars and strategies to prevent diabetes related complications. Please talk to your medical provider for a referral. A co-pay may apply.

Individualized Appointments with a Registered Dietitian and/or a Certified Diabetes Educator
During a one-to-one appointment with a registered dietitian, certified diabetes educator you will receive an individualized diabetes nutrition care plan and disease management education including managing blood sugars and strategies to prevent diabetes related complications. The dietitian will work with you to establish realistic goals based on your medical history and personal preferences that will help you optimize your health, prevent or treat illness and promote positive lifestyle habits. Individualized Appointments with a Clinical Pharmacist
