Diabetes & Heart Disease Prevention

Research shows that healthy eating, moderate exercise, and a healthful lifestyle could delay or possibly prevent Type 2 diabetes and heart disease in people who are at risk. We’ll help you identify the risk factors for heart disease and diabetes that you can easily monitor and manage on your own. This class will provide you with tools to make lifestyle changes including improving diet, increasing physical activity, achieving weight loss, reducing stress, and maintaining healthy blood pressure and cholesterol levels.

Diabetes Education Program

A two-session class which will help you maintain better blood sugar control and learn how to live healthier with Type 2 Diabetes. You will learn lifestyle and nutrition strategies for managing your diabetes, about medications and how to avoid or delay diabetes-associated complications.

Gestational Diabetes

During a one-to-one appointment with a registered dietitian, certified diabetes educator you will receive an individualized gestational diabetes nutrition care plan and diabetes management education including managing blood sugars, learning how to test blood sugars and strategies to prevent diabetes related complications. Please talk to your medical provider for a referral. A co-pay may apply.

Individualized Appointments with a Registered Dietitian and/or a Certified Diabetes Educator

During a one-to-one appointment with a registered dietitian, certified diabetes educator you will receive an individualized diabetes nutrition care plan and disease management education including managing blood sugars and strategies to prevent diabetes related complications. The dietitian will work with you to establish realistic goals based on your medical history and personal preferences that will help you optimize your health, prevent or treat illness and promote positive lifestyle habits. Please talk to your medical provider for a referral. A co-pay may apply.

Individualized Appointments with a Clinical Pharmacist

Healthy Weight Management

A comprehensive program to help you learn healthy eating and physical activity strategies and tips for overcoming weight loss challenges. Topics include food choices, menu planning, portion sizes, dining out, cooking techniques, interpreting food labels, healthy habits and budget-friendly grocery shopping.

Healthy Kids, Teens, and Families 

It’s never too early to teach your children how to lead healthy lives. This class is designed to teach children, adolescents and their families about balanced nutrition and healthy activity, helping them to become healthy kids. Topics include healthy food choices, portion control and label reading, tips for making exercise fun and family communication skills.

Bariatric Education Pre-Surgery Preparation

A comprehensive medically-supervised six-month program to prepare you for weight-loss surgery. Topics include emotional and physical preparation, types of surgeries, risks and outcomes, and long-term lifestyle and dietary changes. Completion is required before surgical consultation is approved.

Bariatric Surgery Support Group

An open support group for you and your loved ones to share your thoughts, be heard and receive on-going support for life before and after bariatric surgery. This is a safe forum where you can discuss the impact of weight loss surgery on your life and learn how to focus on success.

Prenatal & Postpartum Wellness Program

A comprehensive program that will prepare you for baby to arrive. In each class you will learn important strategies to ensure you and your baby are healthy and safe. Topics include the importance of prenatal care, recognizing labor, breast-feeding, proper nutrition for pregnancy and postpartum wellness. This is a four-part series you can join in at any session.

Exercise

Join us for a guided chair exercise routine to improve flexibility, balance, and strength. Open to Facey patients as well as community members.  Space is limited. Participants must pre-register.

Fall Prevention

In this class you will learn to identify the causes of balance problems and how to reduce your risk of falls by learning specific exercises to improve balance. Once you receive a brief fall risk assessment by a physical therapist, we will discuss ways to keep you safer at home.

Advance Directives

In this class you will learn how to complete the California Advance Health Care Directive form. We’ll also discuss various important documents, including: Durable Power of Attorney for Health Care, Living Will, and Physician Orders for Life-Sustaining Treatment (POLST).

Individualized Smoking Cessation Program

This program uses a personal approach to help you identify your readiness to quit smoking, determine your triggers, and assist you with a plan to stop smoking (including medications). You’ll set a quit date and receive resources to become free from your tobacco addiction.

Yoga for Chronic Back Pain

This 15-class series teaches you how to ease the discomfort of lower back pain through the use of yoga stretching techniques. Currently available by physician referral only.
